INTÉRÊT DE L’HYSTÉROSCOPIE ET DE LA COELIOSCOPIE DANS LAPRISE EN CHARGE DU COUPLE INFERTILE :À propos de 200 cas.

But : La stérilité est une pathologie fréquente qui touche près de 10% des femmes en âge de procréation. L’hystéroscopie et la coelioscopie prennent de plus en plus, une place importante dans la prise en charge de cette pathologie. Nous avons essayé d’évaluer les résultats actuels de l’hystéro-coelioscopie, d’en étudier les facteurs pronostiques et les techniques opératoires et de préciser son apport en comparaison avec les autres techniques thérapeutiques.
Méthodes : Il s’agit là d’une étude rétrospective portant sur 200 cas d’hystéro- coelioscopie pour stérilité féminine réalisée au service de gynécologie et obstétrique du centre hospitalo-universitaire Mongi Slim La Marsa.
Résultats: L’hystéroscopie était anormale dans 75% des cas objectivant les synéchies (23%), les hypertrophies (19%), les polypes (10%), les fibromes (9%), l’atrophie (7.5%) et les malformations (3.5%). L’hystéroscopie opératoire a été réalisée dans 27.3% des cas avec un taux de conception global ultérieur de 34.9%.Le taux de discordance entre l’hystérosalpingographie et l’hystéroscopie était de 34.8%. La coelioscopie était opératoire dans 70% des cas répartie entre
adhésiolyse (27.2%), plasties tubaires (37.8%), électrocoagulation ou exérèse de nodules d’endométriose (17.2%) et forage ovarien pour SOPK (17.8%). Le taux de conception global est de 39.9% pour la coeliochirurgie (répartis entre 37.1% de GIU et 2.8% DE GEU) avec une discordance entre les données de l’hystérosalpingographie et de la coelioscopie de 21.1%.
Conclusion : Ainsi l’hystéro- coelioscopie en diagnostiqu ant et traitant les lésions du tractus utéro-tubaire et les pathologies ovariennes tend à être, à la fois, l’examen diagnostique et thérapeutique de première ligne en matière de problèmes d’infertilité féminine avec une discordance avec les données de l’HSG dans plus d’un quart des cas et un taux de grossesse dans plus d’un tiers des cas.
