Etude prospective de l’apport de l’hydrosonographie dans l’exploration de la cavité utérine

Prérequis : Les évolutions techniques récentes de l’échographie ont donné naissance à une nouvelle technique d’exploration : hydrosonographie ou échographie vaginale avec accentuation de contraste (EVAC).But : Évaluer les performances diagnostiques de l’hystérosonographie dans l’appréciation de la cavité utérine, en la comparant à l’échographie endovaginale standard et surtout à l’hystéroscopie diagnostique.
Méthodes : Il s’agit d’une étude prospective s’étalant sur une période de 9 mois allant du 1er janvier au 30 septembre 2009. Toutes les patientes ont bénéficié d’une echographie endovaginale suivi d’une hydrosonographie puis d’une hystéroscopie diagnostique.
Résultats : Soixante deux patientes ont bénéficié des trois examens.L’age moyen de nos patientes était de 42.2 ans. Les troubles hémorragiques du cycle menstruel constituaient les motifs de consultation les plus fréquents. En se référant à l’examen anatomopathologique, l’hystéroscopie s’avère la technique la plus précise dans le diagnostic de l’hypertrophie de l’endomètre et des masses endoacavitaires, suivi de l’hydrosonographie puis de l’échographie endovaginale. La concordance entre l’EVAC et l’HSC dans le diagnostic des fibromes sous-muqueux et des hypertrophies de l’endomètre est très bonne (K>0.8).
Conclusion : L’hystérosonographie parait un examen pertinent, permettant un bilan complet de la cavité utérine.
