Hystérectomie d’hémostase : Indications et pronostic

But : Etudier le profil épidémiologique des patientes, les indications et la morbidité maternelle après une hystérectomie d’hémostase (HH).Méthodes : Étude rétrospective portant sur 46 femmes ayant subi une HH entre 2005 et 2009.
Résultats : L’âge moyen était de 33,3 ans. La parité moyenne était de 3,5. Trente cinq % des patientes avaient un utérus cicatriciel.L’accouchement était par : voie basse (47.8 %), césarienne (52.2 %). L’hystérectomie a été subtotale chez 39 patientes (85 %). Les indications des HH étaient : Inertie utérine (45%), CIVD (26%),
Placenta accreta (8.7%), Placenta praevia (13%), Rupture utérine (8.7%). Trois patientes étaient décédées. Cinq femmes ont présenté une dépression. Six patientes ont présenté une dyspareunie. Cinq femmes ont présenté une diminution de la libido.
Conclusion : L’HH est une chirurgie mutilante entrainant une stérilité définitive. Le développement de l’embolisation artérielle permettra d’éviter le recours à ce geste.
